Substance Abuse Professional (SAP) Evaluations

The Department of Transportation (DOT) regulation 49CFR Part 40 defines Substance Professional (SAP) as a person who evaluates  employees who have violated a DOT drug & alcohol regulation and makes recommendations concerning education, treatment, follow-up testing and aftercare.

 

These evaluations are provided to FMCSA truck drivers, pilots, train operators, vehicle operators, heavy equipment operators & pipeline industry.

 

The SAP Evaluation include the following:

-face to face clinical assessment

-face to face follow up evaluation

-provide referrals to education and/or treatment programs as appropriate

-provide the employer & employee with a written initial evaluation

-provide the employer & employee with a written flu evaluation

-provide the employer with written flu testing recommendations

-provide the employer with recommendations for additional  as appropriate

 

Upon completing the DOT SAP evaluation, the evaluator makes a determination of whether the individual is in compliance with Sap Evaluation commands. The evaluator does not make recommendations about whether or not the individual is ready to return to work.

 

The fee is $800. This includes initial evaluation referral services to appropriate providers monitoring of your case, reporting to your employer and follow-up evaluation. This fee is not covered by your employer & not by insurance.

 

Fee is due at time of service.

Substance Abuse Evaluations

Substance Abuse evaluations is a process to determine if there is an alcohol and/or drug use problem. These evaluations can be part of a court process : such as, driving while impaired, DUI, custody disputes, and employer related situations.

 

Once the evaluation is completed, the evaluator will make a decision regarding the presence of alcohol and/or drug problem and will make individualized treatment recommendations.

 

The fee for the evaluation is $450. This includes initial assessment, diagnosis and treatment recommendations. This fee is not covered by insurance. Payment is due at time of first session.
